Overview
The Medication Management Coordinator under the direction of the Physician or Nurse Practitioner is responsible to perform duties required to complete medication reconciliation process for medical oncology patients. This may be accomplished by contacting outside pharmacies or other healthcare facilities, including processing insurance prior authorization request. Perform clerical functions relating to daily pharmaceutical activities including the use of EMR. This position will work in conjunction to the physician dispensing pharmacy to ensure the medication reconciliation portion of the patient care plan is maintained. The Medication Management Coordinator consistently demonstrates an ability to assess a situation, consider alternatives, and choose the appropriate course of action. Other duties may apply.

Job Skills/Requirements

Previous experience in a clinical setting working with patient medications required. Working knowledge of current drug dosage forms, strengths, generic and trade name traditional and current equivalents. Ability to read, understands, and transcribes pharmaceutical information. Working knowledge of and, the ability to become proficient in, required EMR systems during probationary period. Must have knowledge of computer software, particularly medication management information system software, Microsoft applications (Outlook. Word and Excel). Must possess good organizational, problem solving, written and verbal communication skills for interfacing with all levels of staff, physicians, patients and other contacts. Ability to work effectively with a large diverse population including maintaining good relationships between pharmacy and Nursing Units. Ability to use good judgment in responding to or referring questions by patients and their families, nurses, physicians, and other professional personnel.


Education/Certificate Requirements
Associates Degree or Diploma Program/Certification from an accredited clinical training program and/or certification as a Medical Assistant, Certified Nursing Assistant, Licensed Vocational Nurse, or Pharmacy technician is required.
